A defined group of patients becomes useful once it is examined as a structured review. This pattern sets out the shape a cohort takes as it is built from the record, reviewed case by case, and returned to care as recorded findings.

The shape

A cohort is a defined group of patients selected by shared criteria. A review is a structured examination of that group against a set of questions.

The shape has one entry and one settled exit. It begins with the criteria that define the cohort and ends with findings returned to each patient's care.

Where it starts

The shape starts with the criteria. A reviewer sets the rule that defines who belongs to the cohort, such as a diagnosis, a medication, a length of stay, or a screening due.

Falcon resolves the patients from the record, and each member carries the reason it qualifies. The patient lens applies as the cohort is built, so every patient shown is one the reviewer is entitled to see.
