Record a result
Record a result against an order, enter the values or report, and release it to the patient record where the clinician reads it.
You record a result against the order it belongs to, so the value lands on the right patient and the clinician who ordered it can read it. This page covers entering the values or the report and releasing the result through a second-person check.
Before you begin
A result attaches to the order through a result form. Glyph supplies that form, tied to the test, so the fields match what the order expects.
Two people, two roles
One person enters a result and a different person verifies it. This two-person split is the safety check, and it holds on every result.
Record the result

Open the order on your work list
Find the order and open its result form.
The form opens already tied to the patient and the order.
If the form shows another patient's colour cue, close it and reselect the order.

Enter the values or the report
Fill each field: the measured values with their units and reference ranges, or the written report for an imaging study.
Pickers and built-in calculations guide the entry. The result records as
entered.An out-of-range value shows an abnormal flag as data beside it. If a value sits in the critical range, flag it so it reaches the clinician quickly.

Read back your entry
Check what you typed against the bench or the report.
Correct any field in place before you release it.
If you spot a slip after release, add a corrected version rather than overwriting the first.

Release for verification
A second person, senior to the entry, opens the result and selects Verify.
The result moves to
verifiedand releases to the patient record in Atlas, where the ordering clinician reads it.The verifier is always a different person from whoever entered the result.
The result states
A result carries one of three states, and the state tells you where it stands.

| Entered | You recorded the values by hand. The result waits for a second-person check. |
| Received | The values arrived filled from a connected machine, read-only. The clinician reads them rather than keying them. |
| Verified | A second person confirmed the result and released it to the patient record. |
A correction keeps the first result
An amended result releases as a new version. The earlier one stays on the record, marked superseded, so the history reads exactly as it stood at each moment.
Common questions
Who can verify a result?
A permissioned reviewer, senior to the entry, and a different person from the one who entered it. Pensieve holds the two roles apart at the point of release.
A value is out of range. Does the clinician get told?
An out-of-range value shows an abnormal flag on the result. A value in the critical range is flagged so it reaches the clinician quickly.
How do I correct a result after release?
Add a corrected version. It releases through the same second-person check, and the earlier result stays on the record, marked superseded.
