Record a triage
Record a triage assessment for a patient, capture the vital signs and priority, and place them in the queue for care.
You record a triage on a patient's record in Atlas, the patient record. You capture the vital signs, set the priority, and the patient takes their place in the queue for care.
A triage records a patient's priority for care and the observations behind it. It lives on the patient record, and the emergency view reads the current priority from it.
The priority ladder
The priority picker offers a fixed ladder. Each priority carries a colour and a word, so it reads at a glance on the emergency view.

Record the triage

Open the patient's record
From your active list, tap the patient. The record opens in emergency mode, with the emergency banner pinned above the cards. If the patient holds no triage yet, the banner reads
Untriaged. - 2
Open the Triage card
From the dock at the foot of the canvas, select the Triage card. The card opens with the priority ladder and the observation fields. If the card sits collapsed, tap its pill to expand it.

Record the vital signs
Enter the observations in the fields provided. The values are captured on the card as you go.

Set the priority
Choose the priority from the ladder. The card previews the colour and the word for the level you pick.

Save the triage
Press Set triage. The emergency banner takes the priority's colour and label, and the patient sits in the queue by priority. If the save times out, the card keeps your entry, so press Set triage again.

Update it later
A fresh triage adds a new version and keeps the earlier one. The record shows how the priority changed through the visit.
Each triage is kept
Setting a new priority adds a version rather than replacing the last one. Every triage names the person who set it and the moment they set it, so the record reads clearly under pressure.
Common questions
The banner reads Untriaged. What does that mean?
No triage is set yet. Open the Triage card and set a priority. The banner then shows the colour and the word for that level.
Can I change a priority after I set it?
Yes. A new triage adds a version and keeps the earlier one, so the history stays complete.
Who can set a triage?
A clinician with a current care relationship to the patient. The Triage card opens once you hold that relationship.
