Find a patient

Search Gate for an existing patient by name, identifier or other details, and confirm you have the right record before you continue.

You search Gate to find an existing patient before any other desk task. Search by name or by an identifier, then confirm you hold the right record before you continue. Every visit, order and charge attaches to the record you open here.

Search for the patient

    Choose what you are searching by

    Set the search type: mobile (the default), aadhaar, abha, pensieve id or hospital number. The input adapts to the type you pick.

    Enter the value

    Type the name or identifier into the search box. Results appear as you type. If the value is malformed, the box tells you the shape it expects.

    Read the results

    Each match shows as a tile with the patient's name and key details. Scan the tiles for the person in front of you.

    Open the record

    Select the matching tile. The patient's desk opens, showing their demographics and current visits.

    Confirm the record

    Check the name, the date of birth and a contact detail against the patient before you act. If the tiles hold no fit, register the patient as new.

Confirm before you chart

Confirm the record before you chart against it. The demographics card names the patient, so the visit, orders and charges attach to the right person and the history stays true.

No match found

A search with no match offers an Add New Patient action. Use it for a genuinely new patient, which carries you straight to the registration form. For a patient you expect to find, widen the search: try a different identifier, or check the spelling of the name.

Common questions

Which identifier finds a patient quickest?

A mobile number or an Aadhaar usually returns a single match. A name alone can return several, so confirm against the demographics.

The patient has no record. What now?

Register them. The no-match result carries an Add New Patient action into the registration form.

I found two records for one person. Which is right?

Open each and compare the history. A returning patient keeps one record, so tell an administrator where a duplicate has formed.

Open a visit once you have confirmed the patient's record.
