Hand over a shift

Prepare a shift handover, review each patient and what remains outstanding, and pass a clear summary to the incoming nurse.

You prepare a shift handover from the surfaces you already work in. You review each patient, gather what remains outstanding, and pass a clear summary to the incoming nurse.

A handover draws on three surfaces you know: the active-admissions list, the patient record, and your due list. Together they show who is here and what remains.

Prepare the handover

    Open your ward list

    Open the active-admissions list in Gate, scoped to your patients. Each row shows the patient, the location, the current orders and the time since admission.

    Review each patient

    Open a patient in Atlas and read the record: the diagnoses, the current orders, and the day's events. Work down the list patient by patient.

    Gather what remains outstanding

    Open your due list in Quest: the overdue and due-soon tasks, any missed doses, and orders that wait on someone. You now hold the outstanding items per patient.

    Note what is coming up

    The timeline shows the doses and steps due on the next shift, so the incoming nurse knows what lands first.

    Record the handover summary

    Capture the summary as a handover note on the record. A handover note is a form, and Glyph locks it the moment you finish, so it reads the same to everyone.

    Pass it on

    The incoming nurse signs in as themselves and picks up the same ward list and the same due tasks, now placed with them by the roster.

The work carries between shifts

The record, the orders and the due list carry forward whole. Each nurse signs in as themselves, so every entry names who acted through the shift.

Common questions

Does the incoming nurse see the same list?

Yes. The ward list and the due list follow the roster, so the incoming nurse picks up the patients now placed with them.

Where does the handover summary live?

On the patient record, as a form that locks when you finish. It stays attached to the patient and reads the same to everyone.

What happens to outstanding doses at the shift change?

They stay on the due list with their due times. The next nurse sees the overdue and due-soon tasks at the top.
