Building a form

Before you start

Done.Your role holds a form-design permission.

Design the form

    Open the builder

    In Glyph, open New form. Give it a Name and a short Description that tells staff what the form is for. If a similar form already exists, open it and choose Duplicate, so you start from a known layout.

    Add the fields

    Drag a field from the palette onto the canvas. Choose its type, such as text, number, date, single-select or signature. Set the field Label to the words staff will read. Add each field the form has to capture.

    Mark what is required

    Select a field and set it Required where an entry has to be present. A form completes once its required fields carry a value, so reserve Required for the entries that matter to the record.

    Set validation and calculation

    Give a field a range or a format so an out-of-range entry is caught as it is typed. Add a calculated field to derive a total or a score from other entries. If a rule needs another field first, arrange that field earlier in the layout. Read validation and calculation for the checks a form can carry.

    Arrange the layout

    Group related fields into Sections and set their order. Set a field to appear only when an earlier answer calls for it, so staff see the entries their answers make relevant. If a field sits in the wrong section, drag it to the right one and the order follows.

    Preview and test

    Open Preview and fill a test copy as a member of staff would. Confirm that required fields hold completion, that a range fires when you enter a value outside it, and that a calculated field updates. If a check behaves differently from what you intend, return to the field and adjust its rule.

    Publish

    Choose Publish. The form becomes available to the staff and places you set, and Glyph records this as a version. A later edit publishes a new version, and forms already filled stay bound to the version that was in force when they were filled.

Field types

A field carries a type that sets how staff enter it and how the form checks it.

textfield

Free text, short or long, shown as entered.

numberfield

A numeric entry, open to a range check.

datefield

A calendar date or a date and time.

single-selectfield

One option chosen from a defined list.

multi-selectfield

One or more options chosen from a list.

calculatedfield

A value derived from other fields, held read-only.

signaturefield

A formal sign-off, used on a consent form.

Publishing and versions

A published form carries a version

Each publish records a version of the design. Staff always fill the current version, and a filled form is bound to the version that was in force when it was filled. This is what lets you improve a form over time while every completed copy stays true to the design it was filled against.

Common questions

Can I change a form after staff start filling it?

Yes. You edit the form and publish a new version. Forms already filled stay bound to their own version, so an earlier filled copy reads as it stood.

How do I make one field depend on another?

Set the field to appear only when an earlier answer calls for it, and place that earlier field first in the layout so its value is present when the rule runs.

Where does a published form show up for staff?

A published form reaches staff at the places you set, such as at triage, against an order, or as a step inside a process.
