A form as an order
A clinician orders a form to be filled through Forge, the same way they order a test or a drug. The form is an orderable in the catalogue, so it is picked rather than typed, and the order carries a snapshot of the form as it stood at placement.

A result form on a test
A result form links to the test or study it belongs to. When the laboratory or imaging order is placed, the result form is attached, so the response has a defined place to land. Read laboratory and imaging orders for how a test is ordered and fulfilled.
Because the form is attached to the order, the person filling the result opens it against the test that prompted it. The response returns against that order rather than sitting loose in the record.
The response returns to the record
A completed form lands against its order and against the encounter, in Atlas. A clinician reading the order sees the response beside the request that produced it, so the test and its result read together.
Verified before it stands
A result is entered by hand or received from an external system, then verified by a different person than the one who entered it. This two-person step holds before the result stands as final in the record.
A result carries its provenance
A corrected result is added as a new version, with the prior version kept and marked superseded. Because the result form is bound to its design version and attached to its order, a reader sees which test the result answers, who entered it, and who verified it.
