Ward word and platform word

Staff arrive with the vocabulary of the ward, and the platform maps it onto its own terms. Where the two differ, this glossary gives both. The core concepts page walks one patient event through the main terms in order.

Platform and places

Term	Definition
Pensieve	The hospital platform. One governed record that every tool reads from and writes to.
Organisation	The legal entity that runs the hospital. Holds one or more sites as a grouping label and is the custodian and Data Fiduciary for the care it delivers. It does not own the patient record, which belongs to the patient, exists once across every site that runs Pensieve, and sits outside the tenancy structure with no owning site.
Site	A physical location the organisation operates, such as a main building or a satellite clinic.
Unit	A department or ward inside a site.
Location	A room or bay inside a unit.
Bed	The smallest location, where an inpatient stays.

Record and episode

Term	Definition
Patient record	The durable account of a patient across every encounter. Holds diagnoses, documents, results and consent.
Encounter	A single contact between a patient and the hospital. The ward word is visit or admission.
Episode of care	One encounter seen over its whole length, the span that orders, diagnoses and charges attach to.
Diagnosis	A coded condition recorded during an encounter and carried forward in the record.
Result	A laboratory or imaging finding returned against an order, entered by one person and verified by another.
Document	A scanned or uploaded file attached to the patient record.
Consent	A patient's recorded decision about their own record, held as a first-class entry.
Directive	An instruction a patient gives about who may see or act on their record.
Discharge	The close of an inpatient episode, drawing on clinical readiness and a settled or arranged account.

Orders and money

Term	Definition
Orderable	An item in the catalogue a clinician can request: a drug, a laboratory test, a scan or a consultation.
Order	A specific request for an orderable, placed for one patient inside one encounter.
Ordered snapshot	The frozen copy of an orderable as it stood when the order was placed.
Charge	The billable consequence of an event, priced by the tariff in force when it was raised.
Tariff	A rate card that sets the price of a charge for a payer or scheme.
Invoice	The assembled bill for the charges of an encounter.
Payer	Who settles a bill: a self-paying patient, an insurer or administrator, or a scheme.
Claim	An insurance submission assembled from an encounter's charges.
Deposit	A payment taken in advance of the charges it covers.

Access and safety

Term	Definition
Permission	The right to take a particular action, decided from role and relationship together.
Role	What a person does in the hospital, such as ward nurse or biller.
Relationship	A person's current care connection to the patient in front of them.
Patient lens	The filter applied at the record read path, so a query returns the records a person is entitled to.
Emergency access	A defined path to a record in an urgent case, always logged with the clinician, the time and the reason.
Audit entry	The record that something happened, written in the same transaction as the change it describes.
Two-plane model	Access decided from patient consent and an admission-based gate together.

Interop and desktop

Term	Definition
Connection	A governed link between Pensieve and another hospital system, carried across a controlled boundary.
The bridge	The on-site agent that speaks the protocols of the hospital's machines and carries them over one outbound link.
Chamber	The staff desktop. A shell with a persistent bar, a dock of tools and pinnable widgets.
Dock	The set of tools a person sees, drawn from the actions their role and relationships allow.
Widget	A pinned panel on the desktop that shows a live view from a tool.

The tools

Gate, Atlas, Forge and Vault carry the record, and the tools below carry the model, the access and the daily work around it.

Tool	What it does
Gate	Admissions and appointments, from registration to discharge.
Atlas	The patient record, and the only tool that holds a patient's clinical data.
Forge	Orders, carried through fulfilment, scheduling, billing and results.
Vault	Billing and revenue, from charge to invoice to settlement.
Nexus Manager	The hospital model of places, people, roles and rosters.
Cirith Manager	Access control, deciding each action from role and relationship.
orophin	The on-site bridge to the hospital's own machines.
Westron	The console that configures and monitors the bridge's connections.
Chamber	The staff desktop, with its dock of tools and widgets.
image study viewer	Reads imaging studies in the browser, tied to patient and order.
Cadence	Operational workflows, running a repeatable process step by step.
Quest	Scheduling and due tasks, including medication dose schedules.
Glyph	Forms that staff design, fill and lock.
Sentinel	Alerts, with rules, acknowledgement and escalation.
Raven	Communications over the right channel, within consent.
Falcon	Analytics, with the lineage of every number.
Durin	The builder for a hospital's own tools and widgets.
Vigil	Safety and quality: incident reports, investigations, mortality reviews, complaints and accreditation evidence, with reporter identity held separately so whistleblower anonymity is structural.
