Walks through attaching scanned documents and files to a patient's record and describing each one so it can be found again later.

Scanned documents and files attach to a patient's record and carry a short description, so each one can be found again later. This page walks through attaching a document, describing it, and the kinds of file the record accepts.

    Open the record and go to uploads

    Open the patient's record and choose the uploads area. This is where the record's scanned documents and files sit.

    Choose the file

    Drag the file in, or browse for it. The record accepts images, video and PDF documents, listed below.

    Set the kind

    Mark the file as a document, an image or a video, so it sorts alongside its like.

    Describe it

    Give the file a clear title, such as Old discharge summary 2023, and add any notes. The title is what a colleague searches for later.

    Save

    Save the file. It attaches to the record, carrying your name and the time, and holds as a fixed entry.

Files on an order

A file can attach to a specific order rather than to the record at large. A report or an image returned for a laboratory or imaging order attaches to that order, so it reads beside the request it answers. An imaging study opens in the image study viewer, tied to the right patient and order.

Files on a form

A file can also be the answer to a field on a form, such as a photograph attached to a completed form. The file attaches to that form response, so it travels with the form wherever the form reads. The page on filling and locking covers how a completed form holds its answers.

An upload holds once saved

Once a file is saved to the record, it holds as a fixed entry with its author and time. A replacement is added as a new upload, so the earlier one stays traceable.

Common confusions

What kinds of file can I attach?

The record accepts images, video and PDF documents. The accepted types are listed in the table above.

Where does a report for a laboratory order go?

It attaches to the order and reads beside the request it answers. An imaging study opens in the image study viewer.

Can I attach a file that belongs to no order?

Yes. Ad-hoc files such as old reports and scanned paperwork attach to the record with a title and notes, so they are found again later.
